Which documents do I need to bring with me?

 

Renewing a passport - documents you need to bring:

 

  1. Completed Overseas Renewal Application (PC7) form

  2. Two passport size photographs of yourself – one needs to be signed

  1. Your most recent Australian passport

  • This passport will be cancelled and returned to you with your new passport
  • If you need to travel while your new passport is being processed, you can ask to have your old passport returned to you immediately. However, you must surrender your old passport for cancellation before we give your new passport to you.

 

Applying for a new adult passport - documents you need to bring:

 

  1. Completed Overseas Passport Application (PC8) form 

  1. Two passport size photographs of yourself - endoresed by a guarantor

  1. Proof of your Australian citizenship

  • Your original Australian birth certificate. 
  • Please note that we do not accept birth extracts, commemorative certificates, photocopies (including certified copies) or faxed copies.
  • If you currently do not have your original Australian birth certificate, please contact the Australian Registrar of Births, Deaths and Marriages (RBDM) in your home state to apply for a new document. If you are born after 20 August 1986, and do not have an Australian passport issued after 1 January 2000, please provide proof of one parent's Australian citizenship or permanent residence status in Australia at the time of your birth
  • If you are born outside of Australia provide your valid non-Australian passport or original birth certificate in English.

OR

  • Your original Australian citizenship certificate.
  • This includes Extracts from the Register of Australian Citizenship by Descent, issued to Australian citizens born overseas. If you currently do not have your original citizenship certificate, please contact the Department of Home Affairs.

 

  1. Provide your most recent Australian passport (if in your possession)

  • The passport will be cancelled and returned to you with your new passport.

 

  1. Provide original documents to prove your identity

  • Proof of current name (credit card, health card etc.)
  • Photo identification with signature (current driver’s licence, foreign passport, Australian or foreign residency ID card etc.)
  • Proof of current address (bank statement, utility bill, etc.)
  • Foreign language documents should be translated in English.

 

  1. Provide original documents of any name changes

 

  1. If replacing a lost, stolen, or damaged passport, include completed B11 General Declaration form with a clear statement explaining how, when and where the incident occurred.

 

Applying for a child passport - documents you need to include:

Applicants under the age of 18 must always apply using the Overseas Passport Application Form (PC8). For child(ren) under 16 at least one parent must attend an interview, but the child(ren) does not have to attend. For child(ren) that are 16/17 years of age, both parent and child must attend the interview.  Both parents must consent to the passport being issued and a Guarantor must endorse a photograph of the child and complete section 11 of the application form.

  1. Provide proof of child’s Australian citizenship

  • Child’s original Australian birth certificate showing both parent’s names. It must be presented each time a passport is applied for. The document must be in English, or accompanied by an official translation. If the child is born in Sweden or Finland, you may obtain an Extract of the Population Register in English, as this is the equivalent to a birth certificate. Please note that Finnish church documents are not accepted.
  • If a child doesn’t currently have an original Australian birth certificate, please contact the Australian Registrar of Births, Deaths and Marriages (RBDM) in your home state to apply for a new document.
  • Child’s original Australian citizenship certificate. This includes Extracts from the Register of Australian Citizenship by Descent, issued to Australian citizens born overseas. If you currently do not have your original citizenship certificate, please contact the Department of Home Affairs.
  • If a child is born outside of Australia provide their valid non-Australian passport or original birth certificate in English.

 

  1. Provide child’s most recent Australian passport (if in your possession)

  • The passport will be cancelled and returned to you with a new passport.

 

  1. Provide original documents to prove parents’ identity

  • Proof of parent’s current names (credit card, health card etc.)
  • Lodging parents photo identification (current driver’s licence, foreign passport, Australian or foreign residency ID card etc.)
  • Lodging parent’s proof of current address (utility bill or extract of population document)
  • Parent’s names change document (if applicable)
  • Foreign language documents should be translated in English.

 

  1. Provide parental consent

  • Both parents must give their consent by signing section 17 on the application form. Both signatures must be witnessed.
  • Parental consent: Both parents must have their consent witnessed by a person, who is not related to the child, nor the parents, by birth, marriage, or de facto relationship. The witness must be contactable but does not have to be the same as the guarantor. It can be a friend, neighbour, or colleague. The consent page can be emailed as PDF if the non-lodging parent is overseas. If there has been a name change, we would require a certified copy of the non-lodging parent’s ID to support the consent. Read more about parental consent here.

        8. Swedish extract of population

  • Skatteverket's extract of population (personbevis) may be used to provide proof of residence, particularly for child applications to provide evidence of the mother and father's addresses. To order this, you must call Skatteverket at 077-156 75 67. Please make sure to get the long version in English.
