2. Print, sign and date your form

  • Print the application on plain white A4 paper in black ink only. You may need to adjust your print settings so that the top/bottom of the pages are not cut off. Applications must be less than six months old.

  • All signatures on the application must be in black pen only.

  • Children 10 years or older must sign within the white signature box unless there is a medical reason or disability that prevents them from signing. There cannot be alterations or ‘whiteout’ on the signature or the date. If their signature touches the edge of the box or beyond, the application cannot be processed.

  • All parent(s)/guardian(s) must provide their consent by signing section 15 of the application in front of a witness. Parents are not able to witness each other’s signatures. The witness must be an unrelated third party. Signatures are valid for six months only.

  • guarantor must sign section 11 of the application form and endorse one photo of the applicant (see below).

3. Parents' proof of identity

  • All parent(s) or those with parental responsibility must submit photocopies of identity documents that together show their photo, signature and current residential address. Black and white copies are preferred.

  • If either parent/guardian's current name differs from their name on the child’s birth certificate, please include certified copies of documents that explain the name change. A list of who can certify these documents can be found on our notarials page.

  • Depending on your circumstances, we may require other documents. We will contact you upon receipt of your application to advise if any further documents are required.

4. Photographs and Guarantor

  • Two Australian standard passport photos, less than six months old, with your full name written on the back of one photo with a black pen. The photos must meet our photo guidelines. Please do not attach the photos to the form as this may damage them.

    • Please note: It is helpful if you can provide four photos, in case one or both photos are rejected for minor scratches, dust or other blemishes when scanned. We will return any unused photos to you with your new passport.

  • The back of one photo must be endorsed by the child’s guarantor, by writing ‘This is a true photo of [the child’s full name]’ and then signing it in black pen only.
