Name change certificate
If you want to start your renewal/new passport process you will need to provide original documents of any name changes.
Proof of Name Change - Marriage:

Born and Married in Australia - Marriage certificate issued by the Register of Births Deaths and Marriages.

Born Overseas and Married in Australia - Marriage certificate issued by the Register of Births Deaths and Marriages.

Born in Australia and Married Overseas - Name change certificate or revised birth certificate issued by the Register of Births Deaths and Marriages and the foreign marriage certificate. The foreign marriage certificate will need to be the International version otherwise an official certified English translation is required. If you need a certified English translation please find an Authorised Translator near you.

Born and Married Overseas - Foreign marriage certificate stamped with an Apostille. The marriage certificate will need to be the International version otherwise an official certified English translation is required. If you need a certified English translation please find an Authorised Translator near you. Marriage certificates issued by marriage celebrants or religious leaders are not acceptable to support a name change.

If the documents are in a foreign language then an official certified English translation of all pages is required. If you need a certified English translation please find a Notary Public near you.

Replacement passport
If you change your surname by marriage or revert to a previous surname because of divorce or death of a spouse, you may apply for gratis replacement of a passport that was current at the time of the event. The replacement passport will be issued in your new surname and will have the same expiry date as the current passport. To qualify for a gratis replacement, your current passport must have more than two years validity.
